The answer is D, y - 4 = -2(x - 1).

Point Slope Form: y - y₁ = m(x - x₁)

Coordinates - (1,4)
Slope: -2
   
   (1,4)
   x₁  y₁

Plug slope and point into formula: y - 4 = -2(x -1)
